This report is the result of an independent investigation into the care and treatment of a mental health service user, 'A' in St Helens following a serious incident in 2017.

NHS England/Improvement commissioned Niche Health and Social Care Consulting - who specialise in patient safety investigations and reviews - to carry out an independent investigation into the care and treatment of service user ‘A'. NHS St Helens CCG has been fully involved with the investigation

The main purpose of the independent investigation is to ensure that the mental health care related to service user ‘A’ is investigated in such a way that lessons can be learned effectively to prevent recurrence. The investigation process may also identify areas where improvements to services might be required which could help prevent similar incidents occurring.
